Elderly Self Sufficiency

The EIRHA Elderly Self-Sufficiency (ESS) Program receives funding to hire and maintain a Service Coordinator to allow the elderly and/or disabled Public Housing residents to age in place. The coordinator links residents with supportive services and activities that enable them to make progress towards economic independence and housing self-sufficiency. The coordinator performs many functions, including:

• Recruiting program participants
• Conducting resident needs assessments
• Making referrals to services and programming
• Providing case management and coaching
• Establishing and maintaining partnerships with local service providers
• Tracking outcomes and reporting to HUD
