Elderly Self Sufficiency
The EIRHA Elderly Self-Sufficiency (ESS) Program receives funding to hire and maintain a Service Coordinator to allow the elderly and/or disabled Public Housing residents to age in place. The coordinator links residents with supportive services and activities that enable them to make progress towards economic independence and housing self-sufficiency. The coordinator performs many functions, including:
- Recruiting program participants
- Conducting resident needs assessments
- Making referrals to services and programming
- Providing case management and coaching
- Establishing and maintaining partnerships with local service providers
- Tracking outcomes and reporting to HUD
